I'm going to make a /r/bodybuilding survey. Here are the questions/responses so far. Let me know what you'd like to add/change before I make the survey live

Hey /r/bodybuilding, I'm going to make a long-form census for this sub. I'm wondering what questions you'd like to add/change before I make the survey go up? Most of the questions will be optional so people won't have to answer everything. And please help me with the Supplements section because I don't know a lot about AAS and I don't know what kind of general supplements to include in the "GNC" supplement group. Also, I'd appreciate some app suggestions for tracking food/training. My list isn't very good.

I'm going to make it a requirement to sign in to google just so people don't submit multiple responses, but all responses will be anonymous and emails will not be recorded on my end.
